Security Badge - Request Schedule

The ACP Business Office processes requests for security access cards on Tuesdays & Thursdays, 1:30PM - 4:00PM.

To receive a security access card please follow the instructions below.

Instructions:
1. Complete "Request for Security Access" form and press submit.
2. After submitting, print out the form. Have your supervisor or Investigator sign the form.
3. Bring the signed form to the ACP office (Tuesday and Thursday 1:30PM-4:00PM only).
